What is the True Age Score?

The True Age Score estimates your biological age using real-world health signals—blood markers (like fasting insulin and triglyceride-to-HDL ratio), body composition (waist-to-height ratio), and fitness measures (strength, speed, endurance). It shows how well your body is actually functioning compared to your chronological age. The lower the score, the more youthful and metabolically healthy you are.

How is this different from DNA or telomere tests?

Telomere length and DNA methylation are proxies—indirect markers we hypothesize may reflect aging. They don’t always match your true health status, they change very slowly, and they cost hundreds of dollars. True Age instead measures hard markers of metabolic health—like insulin sensitivity, inflammation, and muscle strength—that reveal how you’re aging right now and give you practical insights to improve.

Brian Sanders

Brian Sanders is the creator of Your True Age, a revolutionary tool for measuring biological health.


A mechanical engineer by training, Brian is also the host of the Peak Human podcast and filmmaker behind the upcoming Food Lies documentary. Over the past decade, he has interviewed and learned from hundreds of the world’s leading doctors, researchers, and PhDs across nutrition, exercise science, human biology, and evolutionary medicine.

Through these conversations and years of research, he uncovered a critical gap: most so-called “biological age” tests rely on static or superficial measures like DNA methylation or telomere length - expensive proxies that tell you little about how your body is actually performing. Brian set out to change that.

Drawing from the best insights in clinical practice, evolutionary biology, and real-world performance metrics, he developed the True Age Score - a dynamic, actionable measure that uses blood markers, body composition, and fitness indicators to reveal how well you’re really aging, and more importantly, how to improve.

Brian is also the founder of the Sapien health movement and Sapien Center in Austin, Texas, where he continues to teach, write, and create tools that empower people to align with nature, restore metabolic health, and thrive for life.
